Dental Labs Exemption Certificate Information

Below you will find exemption information for the state where your dental lab operates. Please click on the appropriate state links for additional information, a blank exemption certificate and a sample of a completed certificate for your reference. Use the sample provided for specific information you will need to fill in to complete the form. You will need to substitute your personal information where indicated and please add your customer number, contact name, and contact phone at the top of the form. A completed certificate will need to be furnished to the tax department to receive a sales tax exemption for applicable product types.
